USA RESIDENTS
IMPORTANT - Federal Law restricts TENS devices to sale by, or on the order of, a physician or practitioner licensed by the law of the state in which he/she practices to use, or order the use, of such devices.
If you wish to purchase a Nidd Valley Medical TENS device or Electrodes we suggest, to avoid complications, that you ask the physician responsible for your health matters to order and pay for it for you. We will then post the TENS unit directly to your physician who will advise you about its use and any contra-indications and precautions relevant to your medical history.
Alternatively you may choose to ignore these restrictions and order a unit on the irrevocable understanding that the manufacturers liability is restricted to repair or replacement of faulty products and that the manufacturer will not accept any responsibility for misuse, misappropriation or accidental misuse or subsequential occurrences as a result of such misuse or misappropriation of the device. The manufacturer assumes that all such responsibilities rest with the user's physician who will provide correct guidance and take whatever precautions are necessary for individual medical conditions. The user is solely responsible for safe use at all times and in all circumstances.
All credit card transactions will be billed in £sterling and converted by your credit card company to your own currency at the prevailing exchange rate (multiply £ by 1.8 as an approximate guide).

VAT Exemption
If you live in the UK and suffer from chronic pain (longer than 3 months without remmision) or are disabled or handicapped you are able to claim VAT exemption on TENS devices by signing a VAT Declaration Order Form.
(For more detailed information please read our 'VAT Facts' Information Sheet). Please note that we cannot accept VAT exemption claim forms by Internet or E-mail or after delivery. This concession must be claimed on the special order form and your order must be sent to us by fax or post.
NOTE - if you are ordering Batteries or Battery Chargers on their own there is no VAT exemption and you must order thesae on the normal VAT Order Form. This is because these items can be used in non-medical equipment. You may claim exemption when ordering these items with the TENS Unit.
To claim exemption you should select the items required and, during the order processing stage, select the 'VAT exempt order form' option.
Complete the form on screen but then print it out and sign the VAT exemption certificate. On completion fax the form with credit card details or post it with your cheque, postal order or credit card details to:
Customer Services, Nidd Valley Medical Ltd., Conyngham Hall, Knaresborough, North Yorkshire HG5 9AY - Fax 01423 799115
